Medical and personal information will be provided to The Cloisters upon admission. Medical information will be on the Agency for Health Care Administration's Health Assessment for Assisted Living Facilities (ALF) form and will include an acknowledgment of the appropriateness of the resident for assisted and Respite Care services. The resident may subscribe to Assisted or Respite Care as long as he/she is considered appropriate for such services.
