Assisted Living and Extended Congregate Care



   A fully licensed ECC facility, staffed with caring professionals whose main focus is the well-being of each resident.

















Extended Congregate Care


   The Cloisters' caring, professional staff provides ECC residents quality care with dignity in elegant surroundings. Residents and familes alike have a secure feeling knowing that our ECC is located on the Garden Terrace floor in our Atrium building within our beautifully landscaped 13 acre gated community. Residents are encouraged to participate in as many of the social and leisure activities as they desire. Our compassionate staff is always on hand to promote feelings of safety and freedom from danger, fear and anxiety. Prior to admission, each indiviual has a medical and personal assessment to determine if ECC services are appropriate. An individualized ECC plan is then created in conjunction with the resident, his/her family members, family physician, The Cloisters' Director of Nursing and our ECC team. During the resident's first thirty days, our conscientious 24 hour staff and Director of Nursing will continue to assess the resident's needs to determine the appropirate level of care.

   For your convenience, The Cloisters provides full rehabilitation services on site (Physical, Occupational, Speech and Respiratory) as preparing for doctor and dentist appointments. Transportation to scheduled doctor's appointments is provided.

            

Upon admission into The Cloisters Retirement Community, medical information must be provided by the applicant's doctor by submitting a "Health Assessment for Assisted Living Facilities" form from the Agency for Health care. This health form will help The Cloisters Director of Nursing determine the necessary level of care and whether the prospective resident is suitable for Assisted Living. The evaluation process will be continued for the first 30 days of residency at The Cloisters in order to ensure the best possible level of care/independence for each individual. On going evaluations will be done quarterly or as necessary.

Assisted Living Suite (Model)

Features private bedroom & full bathroom. All assisted living suites are on the second floor of the Garden Atrium building. This floor is referred to as the Garden Terrace.

Our nurses assist residents to maintain the maximum level of independence.

   

Residents who need minimal assisted care may choose to live independently in a Garden Atrium apartment and contract specific nursing care services for a small, additional monthly fee. Residents who enter The Cloisters as independent living residents always have the security of knowing that a smooth transition can be made into the assisted living program if the need arises in the future.

The following amenities and services are included in the cost of rent:

Nursing Supervision & Emergency Call Response
Scheduling of Appointments & Transportation
The Cloisters nursing staff will monitor resident's activities while on the premises and remind them to carry out activities of daily living. Each unit is wired with our state-of-the-art emergency call system. When activated, the device alerts the nurses to check on the resident. Doctor and dentist appointments are made with the front desk receptionist by The Cloisters staff, a family member, or through an appointment card from a doctor. Transportation to local appointments is provided on Mondays, Tuesdays, and Thursdays.
Medication
Meals & Dining
The Cloisters nursing staff will provide assistance with the administration of prescribed medications. Written instructions must be provided by the resident's doctor. The Cloisters nursing staff will re-order prescriptions and record the administration of all medications. Assisted Living and ECC residents will be served three meals each day in the elegant Garden Terrace dining room. The Cloisters chefs will prepare physician prescribed diets if needed. Tray service to the resident's room is also available. Assistance will be provided with cutting food, pouring beverages, and opening containers.
Bathing
Ambulation
Assistance will be provided three days per week with the following: assembling of bathing items, turning on and adjusting water, helping the resident in/out of the tub or shower, washing and drying, shampooing hair, selection of clothing and dressing. Staff may remain in the suite during bathing. Walking, stair-climbing, and any necessary moving around will be assisted by the nursing staff.
Grooming
Emotional Support
Assistance will be provided with shaving, brusing teeth, cleaning dentures, combing hair, and cleaning, cutting, or filing fingernails. The Cloisters staff will provide assistance in coping with personal care problems to help the resident learn to accept and adjust to changes in physical capabilities and other personal situations. Our caring social service and nursing staff are always available.
Housekeeping
Social & Leisure Activities
Daily housekeeping services will be provided. Personal and linen laundry service (washing, drying, and folding) is provided on a weekly basis. All residents are encouraged to participate in a wide variety of planned activities and programs. See our activities page for details.
Utilities
Electric, basic cable, water, sewer, and pest control are included in all assisted living plans. Telephone service is contracted by the resident.

            

Additional Assisted Living Options

Respite: The Cloisters Assisted Living facility offers short-term care designed not only our residents but also for hire within the surrounding community who have undergone an illness or recent surgery. A furnished one bedroom unit with private bath is provided, along wtih the compassionate care of our 24 hour nursing staff.

Rehabilitation Services:The Cloisters Assisted Living facility features full rehabilitation services on the premises, allowing residents the convenience of never having to leave the comfort of home. We offer physical therapy, speech therapy, and occupational therapy. Our specialists assist post-surgical, fracture, and those who have suffered a stroke to return to the maximum level of independence.


Hospice Care:The Hospice team of dedicated professionals provide pain management and comfort to those who are terminally ill. Medical, social, psychosocial, and spiritual needs of the resident and family are met within familiar surroundings. Hospice services are a covered benefit under Medicare, Medicaid, and most insurance plans.
